This entry could use clarification

A ship that has its hull rating reduced in combat by more than 30% has a chance of exploding equal to the total hull reduction.
So, two things:
a) What kind of a chance? 1%? 10%? 1% per % reduced, what?
b) Does it get re-figured/re-applied each time the ship takes another hit that reduces it still further?
c) OK, so it explodes. That presumably means it is destroyed, but does that have any affect on other ships in the same fleet? Or does it just mean "it got blowed up real good"?
